The dorsal hand skin has fewer sebaceous glands than facial skin, receives significant UV exposure (especially while driving), and is washed multiple times daily — each wash stripping the lipid barrier. The result: hands often look 10–15 years older than the face of the same person.
Why Hands Age Faster
- Low sebaceous gland density — less natural oil production means faster barrier breakdown
- Frequent washing — soap and water strip the lipid barrier; hand sanitizer (alcohol) is even more damaging
- UV exposure — hands receive cumulative UV exposure that most people never protect against; age spots (solar lentigines) are the result
- Collagen loss — the dorsal hand has thin skin with minimal subcutaneous fat; collagen loss is visible earlier than on the face
Nail Biology: What Nails Are and What They Need
Nails are composed of keratin — the same protein as hair. Nail plate hardness depends on water content (optimal: 18%), lipid content, and keratin cross-linking. Brittle nails result from low water content (too dry) or excessive water exposure (repeated wet-dry cycles that disrupt keratin structure). Most nail "strengtheners" work by temporarily hardening the nail plate — not by improving nail health.

🟢 TIER 1 — Confirmed
SPF on hands: Solar lentigines (age spots) are caused by cumulative UV exposure. SPF 30+ applied to the dorsal hands daily prevents new spots and prevents existing ones from darkening. This is the single highest-impact intervention for hand aging.
Occlusives post-wash: Applying an occlusive moisturizer within 3 minutes of washing hands prevents TEWL and barrier breakdown. Tallow, beeswax, and petrolatum are the most effective occlusives for hand skin.
🟡 TIER 2 — Promising
Retinoids on dorsal hands: Topical retinoids improve age spots, skin texture, and collagen density on the dorsal hand. Kang et al. (1995), Journal of the American Academy of Dermatology, PMID 7722028, confirmed tretinoin improved solar lentigines and skin texture on the hands.
Vitamin C for age spots: Topical vitamin C inhibits tyrosinase and reduces melanin production in existing age spots. Apply before SPF.
For Nails
🟢 TIER 1 — Confirmed
Biotin (for deficiency): Biotin supplementation improves nail thickness and reduces brittleness — but only in people with biotin deficiency or brittle nail syndrome. Colombo et al. (1990), Journal of the American Academy of Dermatology, PMID 2273113.
Moisturizing the nail plate: Applying an occlusive to the nail plate and cuticle prevents the wet-dry cycling that causes brittleness. Tallow and beeswax-based balms are ideal — they penetrate the nail plate and cuticle without the solvents in most nail treatments.
🔴 TIER 3 — Limited Evidence
Collagen supplements for nails: Mechanistically plausible (nails are keratin, collagen provides structural support), but human RCT data specifically for nail outcomes is limited.
The Hand & Nail Care Protocol
🌅 Morning
- Gentle hand wash — use a pH-balanced, fragrance-free soap; avoid antibacterial soaps with triclosan
- Pat dry — do not rub

- Retinoid or vitamin C on dorsal hands — apply a small amount of your facial retinoid or vitamin C serum to the backs of your hands before moisturizer
- Rich occlusive layer — Tallow & Honey Balm massaged into hands, cuticles, and nail plates
- Cotton gloves overnight (weekly) — apply a thick layer of balm and wear cotton gloves for 30–60 minutes or overnight for intensive repair
💅 Nail-Specific Steps
- Cuticle oil daily — massage Organic Whipped Tallow Balm into cuticles daily to prevent hangnails and maintain nail plate hydration
- Wear gloves for wet work — dishwashing, cleaning, and prolonged water exposure are the primary causes of brittle nails
- File, don't clip aggressively — use a glass nail file in one direction; back-and-forth sawing creates micro-fractures
- Nail polish breaks — give nails 1–2 weeks polish-free every 2 months to allow the nail plate to rehydrate
Age Spots on Hands: The Treatment Hierarchy
- Prevention: Daily SPF — the only intervention that stops new spots from forming
- Fading existing spots: Vitamin C + niacinamide topically; retinoid at night
- Accelerated fading: Tranexamic acid or azelaic acid for stubborn PIH-type spots
- In-office: IPL, laser, or cryotherapy for established solar lentigines — most effective but beyond OTC scope
