RCT vs. minoxidil 2%: Panahi et al. (2015), SKINmed, PMID 25842469, conducted a randomized, double-blind, controlled trial comparing rosemary oil to 2% minoxidil in 100 patients with androgenetic alopecia over 6 months. Both groups showed significant hair count increases at 6 months, with no significant difference between groups. Rosemary produced significantly less scalp itching than minoxidil. Evidence Tier 1.
Mechanism — vasodilation and DHT inhibition: Rosemary’s active compound, carnosic acid, promotes nerve growth factor (NGF) synthesis and improves scalp microcirculation. Murata et al. (2013), Phytotherapy Research, PMID 22517595, confirmed rosemary extract inhibits 5-alpha reductase (the enzyme that converts testosterone to DHT, the primary driver of androgenetic alopecia). Scalp massage amplification: Scalp massage alone increases hair thickness. Koyama et al. (2016), ePlasty, PMID 27148385, showed that 4 minutes of daily standardized scalp massage over 24 weeks significantly increased hair shaft diameter. Honest limitation: The Panahi et al. RCT used androgenetic alopecia patients specifically. Results for other hair loss types (telogen effluvium, alopecia areata) are less established.
