For longer needles (0.75–1.5mm, typically pen devices), topical anesthesia meaningfully improves compliance and reduces involuntary flinching that can create uneven depth penetration.
Dermaplaning (physical blade exfoliation) causes surface discomfort rather than pain for most users. Numbing is rarely necessary or appropriate for dermaplaning, as the anesthetic can mask over-pressure feedback that protects you from nicking skin.

Lidocaine (2–4% OTC / up to 30% prescription compounded)

Lidocaine is an amide-class local anesthetic that blocks voltage-gated sodium channels, preventing nerve signal transmission. At 2–4% concentrations (standard OTC), it provides mild-to-moderate surface anesthesia with a low systemic absorption risk when applied to intact skin for under 30 minutes.
The FDA issued a 2007 safety warning after two deaths linked to high-concentration compounded lidocaine (23–30%) applied over large body surface areas and occluded with plastic wrap — FDA MedWatch, PMID 17615548. Systemic lidocaine toxicity causes cardiac arrhythmia and CNS seizures. This is not theoretical — it has killed people chasing smoother skin.
Safe use parameters for at-home:
- Max 4% concentration (OTC products)
- Apply only to the face (not large body areas)
- Leave on no more than 20–30 minutes
- Do NOT occlude with cling film or plastic wrap
- Do NOT apply to broken, open, or actively bleeding skin
- Remove completely before beginning any needling procedure
Benzocaine (5–20%)

Benzocaine is an ester-class anesthetic commonly found in OTC tooth pain and oral products. It's sometimes present in numbing creams. The FDA has issued multiple warnings about benzocaine and methemoglobinemia — a condition where red blood cells can't carry oxygen effectively, causing bluish skin, dizziness, and in severe cases, death. Rodriguez et al. (1994), Anesthesia & Analgesia, PMID 8267183. Risk is elevated in infants, G6PD-deficient individuals, and those with cardiovascular disease.
Recommendation: Avoid benzocaine-containing numbing creams for at-home skin procedures. Lidocaine at 4% is a safer alternative for most adults.
EMLA (Eutectic Mixture of Local Anesthetics: 2.5% lidocaine + 2.5% prilocaine)
Evidence Tier: TIER 1 — Gold standard in clinical settings.
EMLA cream is the most well-studied topical anesthetic for pre-procedure use, with a robust clinical record for minor dermatological procedures. Prilocaine carries a low but real methemoglobinemia risk at high doses. In clinical settings it is used under supervision; at home, it's appropriate for adults applying to small facial areas with strict time limits. In many countries it requires a prescription — check your local regulations.
No Numbing (Ice or Cold Compress)
Evidence Tier: TIER 1 — Cold-induced vasoconstriction provides mild analgesia via slowed nerve conduction.
For most at-home dermarollers at 0.25–0.5mm, applying a clean cold compress for 2–3 minutes before needling is often sufficient, carries zero toxicity risk, and has the added benefit of reducing post-procedure erythema. This is the approach we recommend for beginners.
The At-Home Numbing Protocol: Step-by-Step
Step 1: Double cleanse face. Pat dry. No active serums yet.
Step 2: Apply 4% lidocaine OTC cream in a thin, even layer to treatment area. Do not rub in aggressively.
Step 3: Leave uncovered for 20–25 minutes. Set a timer. Do not exceed 30 minutes.
Step 4: Wipe off completely with a clean damp cloth. The numbed skin should feel slightly firm and matte.
Step 5: Proceed with microneedling on clean, product-free skin.

Skip numbing entirely if: Using a 0.25mm roller — cold compress only. If using 1.5mm+ depths, this should not be an at-home procedure.
