Azelaic acid is one of the most underutilized ingredients in skincare. It addresses three of the most common skin concerns — rosacea, acne, and hyperpigmentation — simultaneously, with a safety profile that makes it suitable for sensitive skin, pregnancy, and long-term use. Yet it rarely gets the attention it deserves.

What Is Azelaic Acid?

Azelaic acid is a naturally occurring dicarboxylic acid found in grains like wheat, rye, and barley. It works through multiple mechanisms simultaneously:

  • Antimicrobial: Inhibits Cutibacterium acnes and Malassezia
  • Anti-inflammatory: Reduces reactive oxygen species and inflammatory cytokines — the mechanism behind its rosacea efficacy
  • Tyrosinase inhibitor: Blocks melanin synthesis, reducing hyperpigmentation and PIH
  • Keratolytic: Normalizes skin cell turnover, reducing comedone formation

The Evidence Base

For rosacea: Azelaic acid 15% gel (Finacea) is FDA-approved for rosacea. Thiboutot et al. (2003) in the Journal of the American Academy of Dermatology demonstrated significant improvement in rosacea with 15% azelaic acid gel vs. placebo (PMID 12734493). Evidence tier: Tier 1.

For acne: Azelaic acid 20% cream is FDA-approved for acne. Comparable to benzoyl peroxide and topical antibiotics in mild-to-moderate acne, with better tolerability. Evidence tier: Tier 1.

For hyperpigmentation/melasma: Balina & Graupe (1991) in the International Journal of Dermatology demonstrated significant melasma improvement with 20% azelaic acid, comparable to 4% hydroquinone with fewer side effects (PMID 2071781). Evidence tier: Tier 1–2.

Pregnancy safety: FDA Pregnancy Category B — one of the few active skincare ingredients considered acceptable during pregnancy.

Concentration Guide

  • 10% (OTC): Effective for mild hyperpigmentation, acne prevention, and general brightening. Good starting point for sensitive skin.
  • 15% (prescription in some countries): FDA-approved for rosacea. Meaningful efficacy for all three indications.
  • 20% (prescription): FDA-approved for acne. Most effective for melasma and moderate acne.

The Azelaic Acid Protocol

For Rosacea

For Acne

For Hyperpigmentation

Layering Azelaic Acid

  • Compatible with: Niacinamide (synergistic for brightening), vitamin C (complementary mechanisms), hyaluronic acid, ceramides, SPF
  • Use with caution: Retinol (can be used together but may increase irritation — use on alternate nights if sensitive)
  • Application order: Cleanser → toner (optional) → azelaic acid → moisturizer → SPF (morning)

Side Effects and Tolerance

Azelaic acid is generally well-tolerated, but initial use may cause mild tingling, stinging, or temporary redness — particularly at higher concentrations. This typically resolves within 2–4 weeks. Unlike retinol, azelaic acid does not cause photosensitivity and can be used morning and evening without increased sun sensitivity risk.
